Worksheet D-6 is added to a report along with worksheet D-4 in the Report Settings section.
To get to Report Settings, you can hover over 2552-10 Hospital Cost Report in the Absolute menu bar in the upper left-hand corner. Then you should see a drop-down with the option to select Report Settings.
Then, you can scroll to the bottom and check the box next to the Does this facility operate a sold organ transplant center, or provide cellular therapy acquisition (D-4 and/or D-6) button.
Once the option has been checked, the D Series Organ Acquisition section will be open on the cost report dashboard, which includes worksheet D-6
